- Based on interview and record review, conducted during a site visit on 09/08/23, it was confirmed the facility failed to carry out medication and treatment orders as prescribed. AV did not receive h/h medication on or about 07/15/2023 and the PCP was not notified, ,which is a violation of Oregon Administrative Rules.
