- The facility failed to provide qualified awake direct care staff sufficient in number to meet the 24-hour scheduled and unscheduled needs of each resident in accordance with OAR 411-054-0070(1) per complaint that the facility is constantly short staffed on NOC shift, and staff are not able to adequately assist the residents. The failure is a violation of Oregon Administrative Rules.
