- The facility failed to observe the resident take (e.g., ingest, inhale, apply) the medication unless the prescriber's order for that specific medication states otherwise. Based on facility documentation and interviews, facility staff drop off resident's medication while s/he is asleep and put medications on his/her nightstand in the morning and at night without observing resident took his/her medication. The facility's failure to provide a safe medication administration system is a violation of Oregon Administrative Rules.
