The facility failed to have have qualified awake direct care staff, sufficient in number to meet the 24-hour scheduled and unscheduled needs of each resident in accordance with OAR 411-054-0070(1) per complaint that no staff were visible, supervising residents and a caregiver mentioned that there were not enough people because people had called out sick. The failure is a violation of Oregon Administrative Rules.
