- Based on interview and record review, conducted during a Licensing Complaint Unit site visit (LCU) on May 13, 2024, and May 14, 2024, it was confirmed the facility failed to administer medications as prescribed to Alleged Victim (AV). A review of AV's signed physician orders dated November 12, 2023, revealed an order for dementia medication. A review of AV's MAR dated February 1, 2024, through February 29, 2024, revealed AV missed on dose of his/her medication on February 12, 2024. The facility failed to administer AV's medication as prescribed which is a violation of Oregon Administrative Rules.
