Findings
Staff 9 stated s/he usually worked on the first floor but was sometimes asked to work on the second floor. S/he further stated s/he "did not feel comfortable" on the second floor but "got used to it." The facility failed to provide requested training records. It was confirmed the facility failed to verify direct care staff had demonstrated satisfactory performance in assigned duties.
