- Based on interview and record review, conducted during Licensing Complaint Unit (LCU) during a site visit on May 13, 2024, and May 14, 2024, it was determined that the facility failed to carry out medication orders as prescribed. A review of Alleged Victim's (AV) signed physician order dated February 2, 2024, revealed an order for anxiety medication. A review of AV's MAR dated February 1, 2024, through February 29, 2024, revealed AV missed one dose of the medication on February 2, 2024. The facility's failure is a violation of Oregon Administrative Rules.
