- Based on interview and record review, conducted during a site visit on May 13, 2024, and May 14, 2024, it was confirmed the facility failed to carry out medication orders as prescribed. A review of Alleged Victim's (AV) signed physician orders dated March 19, 2024, revealed an order for Levothyroid once daily. A review of AV's MAR dated March 1, 2024, through March 31, 2024, revealed AV missed one dose of the medication on March 21, 2024. The facility failed to carry out AV's medication orders as prescribed which is a violation of Oregon Administrative Rules.
