Findings
On or about, June 12, 2024, the Licensing Compliant Unit (LCU) confirmed that facility failed to individually identify resident's rooms to assist residents in recognizing their room. The LCU Compliance Specialist observed Resident 2's apartment lacked any identifiable information that would assist Resident 2 with recognizing his/her apartment. The facility's failure is a violation of Oregon Administrative Rules.
