- Based on observation, interview, and record review, conducted during Licensing Compliant Unit (LCU) site visit on May 13, 2024, and May 14, 2024, it was determined that the facility failed to immediately notify the local Department office, or the local AAA, of any incident or abuse or suspected abuse for sampled resident Alleged Victim (AV). AV suffered an unwitnessed fall that was not properly investigated to rule out abuse. The facility's failure to notify local Department office, or the local AAA, of any incident of abuse or suspected abuse is a violation of Oregon Administrative Rules.
