Findings
AP neglected AV as defined in OAR 4110200002 (1) (b)(A)(i) by failing to provide basic care to maintain the health and safety of the AV. W1 dropped AV while transferring h/h by h/h self when AV is a two person assist with gait belt, AV is non weight bearing. As a result of W1 dropping AV, s/h had significant bruising to the left hand and forearm with skin tear. AV had a broken left wist and left wrist sprain.
