Findings
The facility failed to provide adequate supervision according to Alleged Victim’s (AV) care plan for known behaviors and history of altercations. On or about December 25, 2019, AV and Witness 1 were involved in two altercations within a half hour of each other. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
