Findings: The facility failed to provide adequate supervision according to Alleged Victim’s (AV) care plan for known behaviors and history of altercations. On or about December 25, 2019, AV and Witness 1 were involved in two altercations within a half hour of each other. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
