Findings
The facility failed to appropriately care plan and implement meaningful interventions that were effective regarding the Alleged Victim's (AV) falls to ensure safety. Between May and August, AV had approximately 8 unwitnessed falls, some with and some without injury. AV fell on or about August 10, 2020 causing injury to his/her head and knees. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
