Findings: The facility failed to appropriately care plan and implement meaningful interventions that were effective regarding the Alleged Victim's (AV) falls to ensure safety. Between May and August, AV had approximately 8 unwitnessed falls, some with and some without injury. AV fell on or about August 10, 2020 causing injury to his/her head and knees. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
