Findings
The facility failed to follow Witness 1's care plan for 1:1 care and to appropriately monitor him/her due to known behaviors and prior altercations. The failure resulted in a physical altercation with the AV causing him/her unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
