Findings
On or about March 2, 2021, the facility failed to adequately secure the facility which resulted in Alleged Victim's (AV) eloping from the facility. AV was taken to the hospital after he/she was found and was diagnosed with a subdural hematoma. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
