- Findings
- On or about March 2, 2021, the facility failed to adequately secure the facility which resulted in Alleged Victim's (AV) eloping from the facility. AV was taken to the hospital after he/she was found and was diagnosed with a subdural hematoma. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
