Findings
Alleged Victim (AV) relies on the facility for assistance with activities of daily living. A record review of call light response times determined that AV's average wait time for assistance was approximately twenty-three (23) minutes. The facility failed to answer a call light timely, which is a violation of resident rights, is neglect of care and constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(i).
