Findings
Alleged Victim (AV) is care planned to be helped manage appointments and transportation to appointments. AV had two missing appointments on July 5th and 7th, 2021. AV did not get a reminder to his July 23, 2021 appointment. The facility had not scheduled transportation for AV’s July 5th and 7th appointments. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
