Findings: Alleged Victim (AV) is care planned to be helped manage appointments and transportation to appointments. AV had two missing appointments on July 5th and 7th, 2021. AV did not get a reminder to his July 23, 2021 appointment. The facility had not scheduled transportation for AV’s July 5th and 7th appointments. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
