Findings
On or about October 7, 2021, the Alleged Victim (AV) was returned to the facility by Emergency Personnel. AV had eloped from the facility without staff knowledge and had made it 1/2 mile away before falling in front of a dollar store. AV suffered a scratch to his/her forehead from the fall. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
