Findings: On or about October 7, 2021, the Alleged Victim (AV) was returned to the facility by Emergency Personnel. AV had eloped from the facility without staff knowledge and had made it 1/2 mile away before falling in front of a dollar store. AV suffered a scratch to his/her forehead from the fall. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03649 $188.00 fine assessed
