Findings
The Alleged Victim (AV) was prescribed eye drop medication for his/her eyes. From November 8 to November 17, 2021, AV did not receive his/her eye drops as prescribed. Alleged Perpetrator #2 (AP2), Alleged Perpetrator #3 (AP3_ and Alleged Perpetrator #4 (AP4) all documented that they had given AV his/her eye drops, however, there were no eye drops for AV available in the facility, therefore, AV did not receive the medication. AP2, AP3 and AP4 failed to administer the medication or re-order AV's medication, which caused AV to suffer increased eye irritation and puffy, red eyes. AP2, AP3 and AP4's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure to ensure a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse.
