Findings
On or about December 27, 2021, the facility neglected Alleged Victim (AV) by actively or passively failing to provide the basic care and services necessary to keep AV safe when Witness 1 (W1) hit AV and threw coffee at AV. The facility failed to properly care plan and initiate interventions to respond to W1 behaviors, which is a violation to resident rights, is considered neglect of care and constitutes abuse.
