- Findings
- On or about December 27, 2021, the facility neglected Alleged Victim (AV) by actively or passively failing to provide the basic care and services necessary to keep AV safe when Witness 1 (W1) hit AV and threw coffee at AV. The facility failed to properly care plan and initiate interventions to respond to W1 behaviors, which is a violation to resident rights, is considered neglect of care and constitutes abuse.
