Findings
On or about February 12, 2022, the facility failed to follow the Alleged Victim’s (AV) care plan and assist AV with toileting. This resulted in AV being left in urine-soaked clothes for an extended period of time causing a loss of personal dignity and risk of serious harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
