- Findings
- On or about February 12, 2022, the facility failed to follow the Alleged Victim’s (AV) care plan and assist AV with toileting. This resulted in AV being left in urine-soaked clothes for an extended period of time causing a loss of personal dignity and risk of serious harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
