Findings
The facility failed to provide appropriate services according to the AV's needs. The failure resulted in AV being left on the toilet and attempting to self transfer. The AV experienced a fall causing unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
