- The facility failed to provide appropriate services according to the AV's needs. The failure resulted in AV being left on the toilet and attempting to self transfer. The AV experienced a fall causing unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
