Findings
On or about September 07, 2022, Alleged Victim (AV) had seven empty syringes of pain medication missing. Alleged Perpetrator (AP2) (unknown) wrongfully took items belonging to AV, AP2 actions are a violation of resident rights is considered neglect of care and constitutes abuse. The facility failed to protect AV from Financial exploitation which is a violation of Oregon Administrative Rules.
