- Findings
- On or about September 07, 2022, Alleged Victim (AV) had seven empty syringes of pain medication missing. Alleged Perpetrator (AP2) (unknown) wrongfully took items belonging to AV, AP2 actions are a violation of resident rights is considered neglect of care and constitutes abuse. The facility failed to protect AV from Financial exploitation which is a violation of Oregon Administrative Rules.
