Findings
The facility failed to properly care plan for Witness 2’s (W2) known sexual behaviors towards other residents. The failure resulted in W2 inappropriately pinching the Alleged Victim causing pain and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
