- The facility failed to properly care plan for Witness 2’s (W2) known sexual behaviors towards other residents. The failure resulted in W2 inappropriately pinching the Alleged Victim causing pain and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
