Findings
According to the documentation, the facility failed to follow the service plan for the Alleged Victim (AV) when they were ambulating. On or about July 13, 2024, the AV suffered a fall when exiting the facility bus after an outing. The AV’s service plan stated they need standby assistance when ambulating. The AV sustained a scraped elbow due to the fall, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
