Findings: According to the documentation, the facility failed to follow the service plan for the Alleged Victim (AV) when they were ambulating. On or about July 13, 2024, the AV suffered a fall when exiting the facility bus after an outing. The AV’s service plan stated they need standby assistance when ambulating. The AV sustained a scraped elbow due to the fall, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
