Findings
On or about November 3, 2024, the Alleged Victim (AV) called for assistance from facility staff. Alleged Perpetrator #2 (AP2) answered the call light but refused to assist AV and left AV without providing care to AV, leaving AV soiled in his/her bed for hours without assistance. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
