Findings
The Alleged Victim (AV) is dependent on the facility to meet his/her safety needs and has a history of falls. According to an investigation, on or about December 11, 2024, AV fell in their apartment, resulting in skin tears to both arms. AV had last been seen by staff sitting in a recliner in the community living room. AV's service planned interventions were for use of a pressure pad alarm in recliner and bed and for stand by assistance with transfers. At the time of AV's fall, there was only one care staff in the resident living area and that care staff had gone to the restroom. The facility did not have a plan or policy for care staff to provide coverage if a staff needed to use the restroom, resulting in a lack of staff to provide supervision and assistance. The facility failed to ensure sufficient staffing to meet resident needs, which is a violation of resident rights, is considered neglect of care which constitutes abuse.
