Findings
The Alleged Victim (AV) resides at the facility and relies on the facility to meet his/her daily needs. AV's care plan indicates AV dislikes loud noise levels. According to an investigation, on or about February 24, 2025, the Alleged Perpetrator 2 (AP2) failed to protect AV from verbal abuse when AP2 harassed AV by using profanity and making repeated sexual comments to and about AV in a loud tone, resulting in AV becoming upset and requiring transport to the hospital for aggression. AP2's actions are a violation of resident's rights, are considered neglect of care and constitutes verbal abuse. The facility failed to protect AV from verbal abuse, which is a violation of Oregon Administrative Rules.
