Findings
The Alleged Victim (AV) is a known fall risk. On or about March 27, 2025, and March 30, 2025, AV suffered 3 falls. AV was care planned for safety checks 8 times per shift. AV suffered a rug burn and scratches to his/her lower back. The Respondents failure to properly care plan for AV's risk of falls caused AV to suffer a rug burn and scratches. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
