Findings
On or about February 28, 2025, the Alleged Victim (AV) exited the secure facility from an exterior courtyard door. There had recently been a fire drill, and the exterior door mechanism malfunctioned. All doors are to be checked after a fire drill by staff, and while doors were checked, the exterior door was not checked, and AV exited from this door. AV is a fall risk, and leaving the facility without supervision placed AV at risk for harm.
