Findings
The Alleged Victim (AV) relies on the facility for cares and providing a safe environment. According to an investigation, AV suffered at least seventeen falls from 12/5/2025 through 09/17/2025 and at least twelve of those falls were unwitnessed. The facility failed to implement effective interventions to mitigate AV from falling and not all of the temporary service plans written with each fall, nor updated with additional interventions as AV continued to suffer from falls. The facility failed to update the service plan, which is a violation of resident rights, is considered neglect which constitutes abuse.
