- Witness #1 (W1) is known to be verbally and physically aggressive to staff and residents and is known to engage in altercations. W1 requires supervision and monitoring by staff two times per shift and is to have frequent safety checks between those times. On or about December 29, 2025, there was a physical altercation between W1 and the Alleged Victim (AV) where AV sustained injuries, including bruising to his/her left hip, buttock and forehead, displaced ribs, bruised lung and vertebrae fractures. AV was hospitalized for 6 days due to the injuries received in the altercation. Respondent failed to ensure safety of residents and ensure interventions in place were effective in preventing altercations. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
