The Alleged Victim (AV) relies on the facility for care and services. AV suffered multiple falls from 05/09-05/18/2026 which included falls with injuries. The facility failed to implement progressive interventions to AV's care plan to attempt to mitigate the continued falls, which is a violation of resident rights, is neglect which is considered abuse.
