Findings
The Alleged Victim (AV) relies on the facility for their care and is care planned for Staff to provide incontinence care at routine intervals during the day and night. Includes; Checks for wet or soiled brief/disposable undergarment. Staff to assist with ambulation/mobility as needed or requested and document type of assistance provided. According to an investigation, on or about May 10, 2026, AV was found sitting in a heavily urine soiled brief and bowel movement caked to AV's peri-area, resulting in unreasonable discomfort. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care which constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(i).
