Findings: The Alleged Victim (AV) relies on the facility for their care and is care planned for Staff to provide incontinence care at routine intervals during the day and night. Includes; Checks for wet or soiled brief/disposable undergarment. Staff to assist with ambulation/mobility as needed or requested and document type of assistance provided. According to an investigation, on or about May 10, 2026, AV was found sitting in a heavily urine soiled brief and bowel movement caked to AV's peri-area, resulting in unreasonable discomfort. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care which constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(i).
