Assisted Living and Residential Care Facilities must operate and provide services in compliance with all applicable State and local laws, regulations and codes.  This report reflects the findings of the complaint investigation conducted 08/03/2022.  The facility was evaluated for compliance with Oregon Administrative Rule 411, Division 54 and if applicable, Oregon Administrative Rule 411, Division 57.  The following deficiencies were identified:

Based on record review and interview it was confirmed that the facility failed to submit background checks for a criminal fitness determination on all subject individuals. Findings include the following:

During an unannounced site visit on 08/03/2022 Compliance Specialist reviewed Background checks for Staff #2 (S2). S2 had 2 completed background checks the initial dated 12/09/2009 and the second dated 02/16/2012.

In separate interviews with Staff #1 and Staff #3 (S1 & S3) the following was stated:

·I believe S2 worked here through 2019

·S2 was hired in December 2019 and employment ended November 2020.

·The 2012 background check appears to be the last background check we have on file for S2
