Based on interview and record review, conducted during a site visit on 04/03/25, the facility's failure to promptly investigate all reports of abuse and suspected abuse was substantiated for 1 of 1 sampled resident (#3). Findings include, but are not limited to:

In an interview, Staff 1 (Administrator) stated it was reported to him/her that Resident 4 had a missing watch and wedding ring on 01/28/23.

A review of the facility's self report and an electronic communication revealed the incident was investigated by the facility and reported to the Department on 02/03/23.

The findings were reviewed with and acknowledged by Staff 1.

The facility's failure to promptly investigate all reports of abuse and suspected abuse was substantiated.
