The findings of the kitchen inspection, conducted on 08/07/24, are documented in this report. The survey was conducted to determine compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

The findings of the revisit to the kitchen inspection of 08/07/24, conducted 10/29/24 through 10/30/24, are documented in this report. It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

Based on observation and interview, it was determined the facility failed to ensure the kitchen was maintained in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to:

Observations of the facility kitchen, food storage, and food preparation areas on 08/07/24 at 2:45 pm noted the following in need of cleaning or repair:

a. General Kitchen Areas

* The floor drains throughout the kitchen had black and brown build-up;

* Entrance and exit doors had black and brown scuff marks and chipping paint;

* Garbage cans had built-up debris present on the outside and lids;

* Cutting boards, including the ones attached to the steam table and "Fridge 3", had deep grooves and score marks deeming them uncleanable surfaces; and

* The baseboards throughout the kitchen had built up black and brown matter.

b. Front Beverage and Condiment Area

* Reach-in "Fridge 1" and "Fridge 2" had food and beverages that were uncovered and undated;

* Reach-in "Fridge 1" and "Fridge 2" had built up food debris on both the outside of the doors and drips were observed inside of the refrigerators;

* The microwave was missing the handle on the door;

* The juice dispensing machine's back splash had a thick layer of debris that was sticky to the touch;

* The shelves located under the juice dispensing machine had drips and brown matter; and

* The floor to the left of the ice machine had large, circular rust stains present.

c. Front Food Preparation Area

* The underside of the shelf where the heat lamp was located, directly above the steam table, had built-up food debris;

* Inside and outside of the oven and stove had an accumulation of black and brown matter build-up;

* The floor under and around the deep fryer, including the sides of the deep fryer, had a thick layer of grease build-up;

* The hood above the stove had built-up dust and grease present;

* There was dust observed on the ceiling around the air vents;

* To the right of the convection oven, there was a ceiling light cover that was broken; and

* The wall behind the sink had built-up brown matter observed.

d. Janitor Closet

* The door and door jambs were observed to be scuffed, in disrepair, and chipped paint was present; and

* The floor sink had a large crack in it and had brown matter observed.

e. Back Food Preparation Area

* A binder entitled, "Soups and Breakfast Specials" had a thick layer of debris observed on the cover;

* There was a plastic, three drawer storage bin located under the area where the binders were kept that had a thick layer of debris build-up; and

* The industrial mixer had chipping paint.

f. Warewashing Area

* Staff were unaware of which chemical test strips to use for the warewashing machine and were not able to locate the testing strips for the machine;

* The wall underneath the stainless steal shelf to the left of the sink had built up drips and brown matter; and

* The wall behind the sink had black and brown matter around the caulking.

The areas in need of cleaning and repair were reviewed with Staff 1 (ED) and Staff 2 (Health and Wellness Director) on 08/07/24. They acknowledged the findings.

Ordered puma sticks to use for floor drains.  This will be done at cleaning party 08-31-2024 at cleaning party.

Doors to be repainted.  Garbage can were cleaned and build-up removed on lids.  Cutting boards ordered to be replace by re-inspection.

Retraining of staff of 3 day open and date and discard procedures.

Maintenance has fixed large crack and ordered new faucet, and it is replaced.

New binders and plastic drawers. Done 08-24-24

Test strips are not needed for a high temp machine.  Walls will be cleaned during cleaning day and added to task list.  Need maintenance to replace and caulking. Will retain all staff on dishwasher.
