OAR 411-054-0300 (4)(d-i) General Building: Doors-Walls, Cleanable (d) INTERIOR DOORS. Lever-type door handles must be provided on all doors used by residents.(e) EXIT DOORS. Exit doors may not include locks that delay evacuation except as specified by building codes. Such locks may not be installed except with written approval of the Department.(A) Exit doors may not include locks that prevent evacuation.(B) If an electronic code must be entered to use an exit door that code must be clearly posted for residents, visitors, and staff use.(f) WALLS AND CEILINGS. Walls and ceilings must be cleanable in kitchen, laundry, and bathing areas. Kitchen walls must be finished smooth per OAR 333-150-0000 (Food Sanitation Rules).(g) ELEVATORS. An ALF with residents on more than one floor must provide at least one elevator that meets Oregon Elevator Specialty Code (OESC) requirements.(h) The interior of the facility must be free from unpleasant odors.(i) All interior and exterior materials and surfaces (e.g. floors, walls, roofs, ceilings, windows, and furniture) and all equipment necessary for the health, safety, and comfort of the resident must be kept clean and in good repair. This Rule is not met as evidenced by: Based on observation and interview it was determined the facility failed to ensure the interior environment was kept clean and in good repair. The findings include, but are not limited to: During an environment tour of the facility from 01/26/26 through 01/28/26 the following was identified: *Carpet throughout the common use areas were stained; *Public bathroom (near the copy room), carpet at the bathroom door threshold was frayed and the flooring around the bathroom drain was in need of repair; *Service desk near the medication rooms on the first and second floor had gouges and splintered wood rendering the surface uncleanable; *Public bathroom near resident unit 161 had a broken sink faucet and the toilet flushing mechanism had ongoing issues, including previously overflowing; *Rosewood dining room had stained carpet, gouges in the walls and on the floor to ceiling columns throughout the dining room; *Second dining room had a floor threshold near the juice/coffee bar that was separating at the seams, exposing the underneath subfloor, rendering the surface uncleanable; *The handrails in multiple areas throughout the first floor had gouges and splintered wood; *Multiple ceiling tiles throughout the first floor had brown stains; *Multiple ventilation vents throughout the first floor had buildup debris; and *The laundry room walls had small holes in the sheetrock; and *Second floor bathroom sink faucet was broken; drain was rusted. The environment was toured, and the above findings were discussed with Staff 1 (ED) and Staff 6 (Buliding Services Director) on 01/26/26 at 3:00 pm. They acknowledged the findings.

Completed by , building maintenance director, who will continue to monitor, and ensure any replacement handles are compliant as of 01/28/2026. (e)Exit Doors. All locks have replaced to ensure there is no delay in evacuation and (B) push button/coded doors have been identified with clearly posted instructions for residents. Completed by , building maintenance director, who will continue to monitor monthly, and ensure compliance as of 01/29/2026. (f)Walls and Ceilings. Walls and Ceilings are scheduled to be painted to meet smooth/cleanable compliance per OAR 333-150-000 on 03/20/2026, by maintenance assistant. , building maintenance director, will inspect quarterly, and ensure continued compliance. (g)Elevators. All elevators have been corrected to ensure proper operation and lighting to meet OESC completed by , building maintenance director on 02/09/2026, who will continue to monitor, and ensure continued compliance. (h)Interior of the facility must be free from unpleasant orders. (i)All interior and exterior materials and surfaces and all necessary for the health, safety, and comfort of the residents must be kept clean and in good repair. *Carpet throughout the common areas are scheduled for cleaning by Summit Facility Service 03/23/2026. This will be monitored by , building maintenance director, with a quarterly scheduled cleaning, to ensure continued compliance. *Public bathroom near copy room has had new laminate flooring replaced, corrected drainage, and replaced transition strip. Work completed by Abby Floors on 02/03/2026 and will be monitored by , building maintenance director, with a quarterly scheduled inspection, to ensure continued compliance. * Public bathroom near room 161 has had new sink faucet installed and toilet flushing mechanism replaced on 02/06/2026. Will be monitored by , building maintenance director, with quarterly inspection, to ensure continued compliance. *Rosewood dining room carpet scheduled to be cleaned by Summit Facility Service on 03/23/2026. Column dry wall damage to be completed 02/16/2026 by , building maintenance director, with monthly scheduled dining room inspection, to ensure continued compliance. *Second dining room floor transition strip scheduled to be repaired 03/19/2026 by Abby Floors with , building maintenance director, monitoring monthly scheduled dining room inspection, to ensure continued compliance. *Handrails in multiple areas throughout first floor have been sanded, smoothed, and stained to ensure no splintering wood on 02/11/2026. , building maintenance director, will inspect monthly for continued compliance. *Multiple ceiling tiles throughout first floor are scheduled for replacement by 03/25/2026 by , building maintenance director, with continued quarterly monitoring, to ensure continued compliance. *Multiple ventilation vents throughout first floor have been removed and properly sanitized of built-up debris on 01/30/2026 by , building maintenance director. With monthly scheduled dining room inspection, to ensure continued compliance. *Laundry room walls with small sheetrock holes have been repaired as of 01/30/2026 by , building maintenance director, with monthly scheduled inspection, to ensure continued compliance. *Second floor bathroom sink faucet has been replaced and drain repaired on 02/11/2026 by , building maintenance director, with monthly scheduled inspection, to ensure continued compliance.
